Voluntary Life/AD&D

You may purchase voluntary life/AD&D insurance for yourself and your dependents. New York Life Group Benefit Solutions (NYL GBS) administers the plan.

How It Works

Voluntary life/AD&D insurance provides additional financial protection for you and your eligible dependents if you die or suffer a serious injury. You pay the full cost of coverage through after-tax payroll deductions.

If you enroll in voluntary life/AD&D within 31 days from your date of hire, you may purchase up to the guaranteed issue amounts and not be required to provide evidence of insurability (EOI). If you don’t enroll when you are initially eligible, or you purchase coverage above the guaranteed issue amounts, you will be required to complete an Evidence of Insurability Form and return it to the Stimson Benefits Department.

Designate a Beneficiary

Go to UltiPro to list the beneficiary or beneficiaries who will receive your insurance benefits.

Eligibility

You are eligible for coverage if you are an active, full-time hourly or salaried employee who regularly works at least 30 hours per week.

When Coverage Begins If you enroll within 31 days from your date of hire and purchase the guaranteed issue amount, coverage takes effect the first of the month following your hire date. Otherwise, coverage takes effect once your Evidence of Insurability (EOI) application is approved by New York Life Group Benefit Solutions.
Benefit Depending on the voluntary life insurance amount you elect, you, your spouse and children will receive an equal amount of voluntary AD&D insurance.
Employee Benefit Amount
  • Increments of $10,000
  • Maximum benefit is five times your covered annual compensation or $500,000 (whichever is less)
  • New hires: Guaranteed issue of up to $100,000, if enroll when initially eligible and no evidence of insurability (EOI) is required
Spouse Benefit Amount
  • Increments of $10,000
  • Maximum benefit is 100% of your voluntary life benefit amount or $500,000 (whichever is less)
  • New hires: Guaranteed issue of up to $30,000, if enroll when initially eligible and no evidence of insurability (EOI) is required
Child(ren) Benefit Amount 
  • Increments of $2,500
  • Maximum benefit is $10,000 ($1,000 for a dependent child who is less than six months old)

Claims should be reported within 31 days of the date of loss and “proof of loss” within 90 days. Call 888-842-4462 with questions.

Continuation of Coverage

If you leave Stimson, you can elect to continue your coverage directly with New York Life Group Benefit Solutions by completing the following:

Your annual wage or salary as reported by Stimson for work performed for Stimson on the date the covered loss occurs; it does not include bonuses, commissions, overtime pay or other extra compensation.
