Eligible Compensation
Your total salary or wages paid to you as shown on your W-2, excluding fringe benefits, reimbursements or other expense allowances, moving expenses, deferred compensation, welfare benefits, overtime, bonuses, commissions and amounts in excess of your annual base compensation.
Weekly Earnings
Your weekly wage or salary just prior to the date of disability; it doesn’t include bonuses, commissions, overtime pay or other compensation.

Your annual wage or salary just prior to the date of your disability; it does not include bonuses, commissions, overtime pay or other extra compensation.
