Death
If your dependent dies, contact Human Resources to help you through the process. While this is a sensitive time, you may need to make changes to your benefits.

What You Need to Do

Make benefit changes within 31 days after your dependent’s death on UltiPro. From the menu, select the “Myself” tab, then “Life Events.” Changes will take effect the first of the month following your dependent’s death. For example, if the death occurs November 7 and you make your changes November 12, benefit changes take effect December 1.

Changes You Can Make
Medical, Dental and Vision
  • Drop your dependent coverage.
  • If you were previously enrolled in your spouse’s coverage, you may enroll yourself and eligible dependents in a Stimson plan.
Health Savings Account
  • Enroll or drop HSA coverage.
  • Increase or decrease your contribution amount.
  • Make your changes on UltiPro. Select “HSA Change Life Event.”
  • Update your beneficiaries on the PayFlex website.
Health Care FSA
  • Change your FSA election (increase, decrease, start or stop) as long as the change is consistent with your change in status.
  • Make your changes at UltiPro.
Dependent Care FSA
  • Change your FSA election (increase, decrease, start or stop) as long as the change is consistent with your change in status.
  • Make your changes at UltiPro.
401(k) Plan
  • Enroll or change your contribution anytime during the year on Fidelity NetBenefits or by calling 800-835-5097.
  • Update your 401(k) beneficiary on Fidelity NetBenefits.
Basic Life / AD&D
  • File a life insurance claim by completing the Life/AD&D Claim Form and submitting it to the Stimson Benefits Department.
  • Update your beneficiaries at UltiPro.
Voluntary Life / AD&D
  • Change coverage amounts for yourself and/or surviving dependents. (Evidence of insurability is required to increase coverage.)
  • Update your beneficiaries.
  • If your dependent was covered, initiate the claim by completing the Life/AD&D Claim Form.

Bereavement Leave

Take up to 24 hours of paid bereavement leave for the death of an immediate family member.

Note: Under the Oregon Family Leave Act, you may take two weeks of bereavement leave within 60 days of the date you received notice of the covered family member’s death.

Personal Leave Time off is available for special circumstances not covered by any other leave policy.

Username: STMSN plus 6-digit clock number. If clock number is less than 6 digits (e.g., 1234), add zeros to ensure a total of 6-digits: STMSN000123.
